ENROLLMENT POLICIES

ENROLLMENTREQUIREMENTS: PSA Birth Certificate Good MoralForm 138 (School Report Card)Promissory note (for incomplete school forms or PSA birth certificate)Parent/Guardian Barangay ClearanceParent/Guardian two valid IDs.

STEPS: Accomplish Enrollment FormFollow steps/instruction for enrolment:- Requirement Checking / Verification- Physical Examination- Assessment (Reading and Comprehension)- Interview Successful enrollees will received a slip​with designated section ​

TRANSFER-IN
 
REQUIREMENTS:Form 138 (School Report Card)Good MoralCertification of EnrollmentPSA Birth Certificate​Promissory note (for incomplete school forms or PSA birth certificate)Parent/Guardian Barangay ClearanceParent/Guardian 2 valid IDsWritten letter indicating the reason of transfer ​
STEPS: Go to designated Curriculum ChairmanFill out Enrollment FormInterview (Parent and Student)Will received an endorsement slip submitted to the Registrar.Successful enrollee will be given a designated section and will report the next day 

TRANSFER - OUT

REQUIREMENTS:Accomplished activities, examination, output per subjectRequest Letter for Transfer indicating the reason of transfer, name and contact of transfer school Accomplished School Clearance

STEPS:Submit Request Letter for Transfer with parent's signature to your adviserAsk subject teachers about tasks to be accomplishedGet a School Clearance on Records; when accomplishedsubmit to the Registrar Successful transferred-out will received Form 138(Student's Report Card) and Good MoralNOTE:This is NOT a ONE DAY PROCESS. Duration depends on accomplishment of requirements given by subject teachers and accomplishing the SCHOOL CLEARANCE.
